This Waiver applies to all services provided by Endless Summer Swim School LLC, including but not limited to swimming lessons (individual or group), open swim sessions, birthday parties, private instruction, and any other water-based activities at private residences, pools, or other locations during at-home visits or traveling services.
Participation in swimming and water-based activities involves inherent risks including drowning, slips and falls, physical exertion injuries, muscle strains, impacts, allergic reactions to water treatments, and other aquatic hazards. These risks may result in serious injury, permanent disability, or death. Participant confirms good health or has consulted a physician if needed.
Participant certifies no undisclosed medical conditions that increase risk. Staff may administer basic first aid and seek emergency medical care if necessary at Participant’s expense.
Participant voluntarily assumes all known and unknown risks associated with participation.
Participant releases Endless Summer Swim School LLC, its owners, employees, instructors, contractors, and affiliates from any and all liability arising from participation, including claims based on negligence, to the fullest extent permitted by Florida law.
Participant agrees to indemnify and hold harmless the Released Parties from claims related to participation.
Parents/Guardians must remain on premises and supervise minors outside instructional time.
Permission granted for promotional photography and video use.
Valid for one (1) year from signing unless revoked in writing.
Governed by the laws of the State of Florida. Venue in Leon County, Florida.
If any provision is invalid, remaining provisions remain effective.
Electronic or printed signatures are legally binding.
